This is a dream Case Management position..no on call, no weekends, no holidays, no staff oversight...which allows you to have the time and ability to advocate for those in need. Cerebral Palsy of Southwestern Illinois is a small, stable, non for profit agency located in Fairview Heights, IL. We strive to ensure individuals diagnosis intellectually and developmental disabilities and their families maintain as much independence as possible. Looking for a QIDP/ Case Manager/SDA for our Home and Community Based Services Program, serving individuals with Disabilities. QIDP eligible requires a Bachelors Degree within the human services field and one year experience with caregiving. Will train and certify the right candidate. Typical business day hours, with occasional early evening hours based on the individuals' needs and availability. Initially, 5 days a week Monday - Friday during training, once training is complete it is a flexible four-day week work week (8 hours a day).

Duties are to Advocate for individuals served, coordinate home visits with individuals and guardians to assess choices and satisfaction, coordinated needed changes, complete required records, visiting reports, and other related regulatory documentation, coordinate with outside agencies and supports. Requires organization and independence in scheduling time efficiently.

JOB QUALIFICATIONS:

- BA degree in the Human Services or Related field (Criminology, Sociology, etc. also accepted).

-At least year working with individuals with disabilities (this can be paid or unpaid service)

- Must meet Qualified Intellectual Disability Professional (QIDP) qualifications. Will provide QIDP certification for the right candidate.

-A personal vehicle that is insured and driving record/license in good standing that meets insurance standards

-Effectively utilize Outlook, Word, Calculator for basic budgetary calculations Copy, Fax, Scanner, Zoom, etc

-Effectively pass and become certified in CPR/First Aid for Children and Adults (will provide training).

-Ability to meet Essential Job Functions with or without reasonable accommodations.

SUPERVISORY RESPONSIBILITIES: no

TYPICAL WORKDAY: Full-time exempt salary employee; Mon-Fri day hours

PHYSICAL QUALIFICATIONS

Lift 25 pounds.
Fine motor skills to operate office equipment, write, and type.
Valid Driver’s license, personal auto insurance, maintain a good motor record, and maintain a personal vehicle.
Ambulate in the community.

SDA assists the individual (or the individual's family or representative, as appropriate) in arranging for, directing and managing services within our Home and Community Based Program offered here at Cerebral Palsy of Southwestern Illinois. It also offers practical skills training to enable individuals and families to independently direct and manage Waiver services. SDA includes providing information to ensure that individuals understand the responsibilities involved with directing their services. In general, the activities provided under SDA must be consistent with Federal CMS regulations.
