Job Overview

This position involves conducting comprehensive health assessments in a variety of settings, including home visits, skilled facilities, and telehealth. The successful candidate will have the opportunity to make a direct impact on the health and well-being of others while working in a flexible, PRN/part-time schedule.

  • Responsibilities:
  • Conduct adult/geriatric assessments, medical history, diagnosis, treatment, health education, physician referrals, case management referrals, follow-up, and clear documentation according to Matrix guidelines and protocols.
  • Collaborate with physicians, case managers, social workers, family members, key caregivers, and ancillary medical personnel as appropriate.
  • Provide patient education and follow-up services in collaboration with Primary Care Physicians (PCPs).
  • Deliver services in various venues, including home visits, skilled facility visits, and telehealth visits.

Qualifications

  • Requirements:
  • Master's degree required or commensurate experience and satisfactory completion of NP licensure.
  • Current RN and NP or PA licensure in the state of practice, including prescription authority or ability to obtain prescriptive authority.
  • Board certification by NCCPA, AANP, ANCC, or AACCN in a Matrix-approved specialty.
  • Current BLS, ACLS, or CPR certification.
  • Minimum 1 year experience as a Nurse Practitioner/Physician Assistant.
  • Ability to travel within the state or out of state.
  • Strong computer skills and familiarity with EMR software.
  • Excellent verbal and written communication skills.
  • Positive and outgoing personality, with a strong team player mindset.
