Job description

The Nurse Practitioner provides comprehensive medical evaluation, education and management for clients that present for routine, preventative, acute and chronic disease health care services at the clinic site.
The nurse practitioner will coordinate all of the services necessary for health promotion, maintenance, rehabilitation and the prevention of disease and disability.

Qualifications:
Must hav
e peds experience
Licensed to practice as a NYS Nurse Practitioner.
Licensed and currently registered as a Registered Nurse in the State of New York.

Responsibilities:
Assesses and interprets medical and psycho-social histories; performs physical
examinations (including gynecological and breast examination as appropriate; identifies problems; formulates a plan of treatment; provides education and referral (as indicated); and evaluates the effectiveness of the interventions prescribed.
Consults with or refers clients with abnormal conditions or other medical, psychological or social problems to the collaborating physician or other appropriate referral course when indicated.
Performs or orders laboratory tests and interprets the results as part of the health management.
Reports communicable disease in a timely manner to the appropriate Health Department as per health center protocol.
Orders medications.
Follows established treatment guidelines as indicated by Health Center protocols.
Conducts health teaching and counseling during the medical encounter, as appropriate, enabling the client to make informed choices about health and treatment options.
Completes the client’s clinical record documentation in a timely manner specific to the applicable policies and procedures of the health center.
Understands the medical emergency procedures and is familiar with the location of the emergency kit/supplies in all clinic settings.
Participates in quality improvement/assurance activities.
Participates in continuing medical education activities to enhance professionalism and to ensure quality of care.
Provides on-call coverage as scheduled.
Works collaboratively with all members of the health center to build a supportive team.
Applies infection control measures according to Health Center Policies and Procedures.
Maintains professionalism and adheres to client confidentiality.
